Position: Social Worker
MSF is seeking a qualified Social Worker to support vulnerable individuals through case management, psychosocial support, and referral services. The role involves assessing needs, identifying risks, and developing tailored intervention plans to improve the well-being of individuals in distress.
Key Responsibilities:
- Conduct thorough case assessments to identify vulnerabilities and critical social needs
- Perform risk assessments and develop intervention plans
- Provide direct support or refer individuals through safe referral pathways
- Collaborate with healthcare providers on multidisciplinary case management
- Build and maintain referral networks with community and institutional partners
- Ensure proper follow-up and case closure
- Promote patient participation and informed decision-making
Requirements:
- Bachelor’s Degree in a relevant field
- Minimum of 2 years experience in social work or related roles
