POSITION: REFERRAL NURSE

ABOUT LIFE HELPERS INITIATIVE (LHI)
Life Helpers Initiative is a Nigerian national non-governmental organization established in 2004, with operations across multiple states including Borno, Yobe, Adamawa, Bauchi, Plateau, Sokoto, Kebbi, Katsina, Zamfara, Ebonyi, and the FCT.
The organization works to empower children, youth, women, and vulnerable communities through interventions in:
- Health
- Education
- Livelihoods
- Gender & Social Inclusion
- Agriculture
LHI is currently implementing a UNOCHA-funded humanitarian project in partnership with Save the Children, Greencode, and EYN. The intervention focuses on life-saving health and nutrition services for women and children affected by acute malnutrition in Borno and Yobe States.
Key activities include:
- Mass MUAC screening
- Infant and Young Child Nutrition (MIYCN)
- Tom Brown nutrition support
- Management of acute malnutrition cases
- Community-based health and nutrition interventions
POSITION OVERVIEW
The Referral Nurse will play a key role in strengthening the health referral system within the project areas. The role ensures that patients—especially vulnerable women and children—are properly identified, referred, and tracked across different levels of healthcare services.
The officer will work closely with health facilities, community volunteers, and project staff to ensure continuity of care and timely access to treatment services.
KEY RESPONSIBILITIES
- Facilitate timely and appropriate referral of patients to health facilities
- Coordinate with community structures and health providers for case identification and referral
- Maintain accurate documentation of all referred cases and follow-up actions
- Track referred patients to ensure continuity of care and treatment completion
- Support basic clinical care services where necessary under approved guidelines
- Strengthen linkage between communities and health facilities
- Participate in case management activities, especially for malnutrition and maternal/child health cases
- Conduct follow-up visits and ensure proper reporting of patient outcomes
- Collaborate with project teams to improve referral efficiency and service delivery
- Contribute to health education and awareness within communities
REQUIRED QUALIFICATIONS
Education
- Bachelor’s Degree or Diploma in Nursing (BSc Nursing, RN, or equivalent)
- Must possess a valid license from the Nursing and Midwifery Council of Nigeria
Experience
- 2–3 years of clinical nursing experience
- Experience in primary healthcare, maternal and child health, or nutrition programs preferred
- Prior NGO or humanitarian experience is an added advantage
- Experience working in Northern Nigeria, especially the Northeast, is desirable
Skills & Competencies
- Strong knowledge of referral systems and patient case management
- Good documentation, reporting, and data management skills
- Proficiency in Microsoft Office (Word, Excel)
- Strong communication and interpersonal skills
- Ability to work in remote and resource-limited environments
- Familiarity with Hausa and/or Kanuri languages is an advantage
- Strong commitment to safeguarding, accountability, and humanitarian principles
- Ability to work independently and within multidisciplinary teams
